12 Days of Giveaways

We don’t know about you, but we are ready to put 2020 behind us and start 2021 off with a fresh start! We know how hard you’ve been working both professionally and personally to hold it down! So let’s kick those COVID blues away and have some fun with ​12 Days of Giveaways!

When​: ​Giveaways will be starting on December 8th to December 19th, but you can start now by entering your name for the daily giveaway. Just fill out the form below and you’re entered.

What​:​ Once a winner for that given day has been chosen, we will give them the option to pick the prize they want. Once that prize has been picked, it will no longer be available to the other winners.

*Must respond within 24hrs to claim the prize that is desired.

Who​: Contest is open to everyone that is local! You do not have to be a patient to win a prize, we just ask that you pick up your prize at one of our office locations (Collegeville or Gilbertsville) No prizes will be mailed. Winner will be contacted through the given email address or phone number that is on file (if you are a patient.) Simply use the contact form below and enter your name and email address. And that’s it, you’re entered!
